Click on this link, print the form and complete it to renew your CTF membership. Then give it to your instructor or mail it to CTF Headquarters, P. O. Box 10, Deer, AR 72628. The annual membership cost is only $25.00. The CTF will then mail you your current membership card and a new CTF patch to your Taekwondo school.

APPLICATIONS FOR BLACK BELT TESTING

IF YOU ARE TESTING AT A REGIONAL OR NATIONAL BLACK BELT TESTING, these forms are due at CTF Headquarters 10 days prior to the scheduled testing date.

  Black Belt Testing Application

This form must be completed by all students testing for any Black Belt Rank, including 1st Black Recommended. It must be turned into your instructor for approval prior to testing.

Red Collar Testing Application

 - Complete this form if you are testing for Assistant Instructor. Submit it with a current First Aid and CPR certification.

Black Collar Testing Application

 - Complete this form if you are testing for Certified Instructor. Submit it with a current First Aid and CPR certification.
